Thyroid diseases are more prevalent in women, and this difference seems to be associated with the oxidative stress found in the thyroid of females. Thyroid NADPH Oxidase 4 (NOX4) was shown to respond to estrogen, which can also modulate TGF-ß1, a potent stimulator of NOX4. This study aimed to investigate the effects of TGF-ß1 on redox homeostasis parameters in the rat thyroid cell PCCL3 and the interrelationship between estrogen and TGF-ß1. TGF-ß1 treatment increased both intra- and extracellular ROS generation along with NOX4 expression and reduced GPX and catalase activities, extracellular H2O2 scavenging capacity, and reduced thiol content. TGF-ß1 mRNA and protein expression are higher in female thyroid glands of rats in comparison to males. Moreover, 17ß-estradiol treatment enhanced TGF-ß1 mRNA in PCCL3 cells, decreased extracellular bioavailability but did not activate Smad pathway. Our data suggest that higher levels of TGF-ß1 in females are potentially related to higher ROS availability which may be associated with the sex disparity in thyroid disorders.

The use of ß-galactosidase in food products has been a major focus of the industry. Therefore, the development of efficient and inexpensive methodologies to purify it is essential. Thus, this study aimed to recover the enzyme ß-galactosidase (ß-gal) by ion-exchange chromatography in a fixed-bed column. Batch adsorption tests were performed using four types of adsorbents. The ß-gal adsorption capacity in batch mode using Streamline DEAE resin presented the best performance, with a retention capacity of 18.77 ± 0.14 U/g at pH 6.0. A 22 experimental design was applied to optimize the ß-gal recovery using an AKTA Start system, evaluating the ionic strength and the pH as process parameters. The results showed that ionic strength exerted a greater influence on fold purification (FP). The ß-gal fraction in elution using 0.1-0.4 M of NaCl showed a yield of 51.65 ± 0.17% and FP of 2.00 ± 0.43. Electrophoresis confirmed the ß-gal recovery, where an evident band with a molecular weight between 60 and 120 kDa was observed. These results point to the recovery of a stable ß-gal of K. lactis with potential industrial applications.

OBJECTIVES: HIV outcomes centre primarily around clinical markers with limited focus on patient-reported outcomes. With a global trend towards capturing the outcomes that matter most to patients, there is agreement that standardizing the definition of value in HIV care is key to their incorporation. This study aims to address the lack of routine, standardized data in HIV care. METHODS: An international working group (WG) of 37 experts and patients, and a steering group (SG) of 18 experts were convened from 14 countries. The project team (PT) identified outcomes by conducting a literature review, screening 1979 articles and reviewing the full texts of 547 of these articles. Semi-structured interviews and advisory groups were performed with the WG, SG and people living with HIV to add to the list of potentially relevant outcomes. The WG voted via a modified Delphi process - informed by six Zoom calls - to establish a core set of outcomes for use in clinical practice. RESULTS: From 156 identified outcomes, consensus was reached to include three patient-reported outcomes, four clinician-reported measures and one administratively reported outcome; standardized measures were included. The WG also reached agreement to measure 22 risk-adjustment variables. This outcome set can be applied to any person living with HIV aged > 18 years. CONCLUSIONS: Adoption of the HIV360 outcome set will enable healthcare providers to record, compare and integrate standardized metrics across treatment sites to drive quality improvement in HIV care.

Vitamin D is a fundamental regulator of host defences by activating genes related to innate and adaptive immunity. Previous research shows a correlation between the levels of vitamin D in patients infected with SARS-CoV-2 and the degree of disease severity. This work investigates the impact of the genetic background related to vitamin D pathways on COVID-19 severity. For the first time, the Portuguese population was characterized regarding the prevalence of high impact variants in genes associated with the vitamin D pathways. This study enrolled 517 patients admitted to two tertiary Portuguese hospitals. The serum concentration of 25 (OH)D, was measured in the hospital at the time of patient admission. Genetic variants, 18 variants, in the genes AMDHD1, CYP2R1, CYP24A1, DHCR7, GC, SEC23A, and VDR were analysed. The results show that polymorphisms in the vitamin D binding protein encoded by the GC gene are related to the infection severity (p = 0.005). There is an association between vitamin D polygenic risk score and the serum concentration of 25 (OH)D (p = 0.04). There is an association between 25 (OH)D levels and the survival and fatal outcomes (p = 1.5e-4). The Portuguese population has a higher prevalence of the DHCR7 RS12785878 variant when compared with its prevalence in the European population (19% versus 10%). This study shows a genetic susceptibility for vitamin D deficiency that might explain higher severity degrees in COVID-19 patients. These results reinforce the relevance of personalized strategies in the context of viral diseases.Trial registration: NCT04370808.

We report a case of a man with chronic hepatitis C infection treated with remdesivir for COVID-19, resulting in lowered HCV viral load, followed by a rebound after its discontinuation. Concomitant treatment with tocilizumab possibly caused loss of anti-HBs.

Organizing pneumonia (OP) is a sub-acute process of pulmonary tissue repair secondary to lung injury, defined histopathologically by intra-alveolar buds of granulation tissue within the lumen of distal pulmonary airspaces. It can be either cryptogenic or secondary (SOP) to different clinical conditions, namely infections. Despite being nonspecific, its diagnosis can be made by the association of clinical and imaging criteria. We report two cases of OP associated to SARS-CoV-2 pneumonia, admitted at a Portuguese tertiary hospital unit dedicated to COVID-19. Both patients presented with severe respiratory failure with need of invasive mechanical ventilation. After initial recovery, there was worsening of dyspnea and hypoxemic respiratory failure with increase in inflammatory markers. Chest CT revealed an OP pattern. Other conditions such as superinfection, auto-immune disease and iatrogenic etiology, were excluded and corticotherapy at a dose of 1 mg/kg/day was administered. Chest CT follow up of both our patients showed complete resolution of OP pattern, with mild to moderate residual pulmonary fibrosis without honeycombing. There is no OP to SARS-CoV-2 case series yet published describing the progress of patients after corticotherapy, although the association between systemic corticosteroids and lower all-cause mortality in patients with COVID-19 has been recently established. It is possible that, as has been described with other viruses, OP secondary to SARS-CoV-2 represents an immunological process after initial infection, presenting with elevation of inflammatory markers and cytokines storm in the bloodstream and lung tissue, which may explain the favorable response to corticosteroids.

The present study aimed to evaluate the lactose hydrolysis conditions from "coalho" cheese whey using ß-galactosidase (ß-gal) produced by Kluyveromyces lactis immobilized with sodium alginate. Three sodium alginate-based immobilization systems were evaluated (0.5, 0.7, and 1% w/v) for maximizing the immobilization yield (Y), efficiency (EM), and recovered activity (ar). The lactose hydrolysis capacity of the immobilized form of ß-gal was determined, and simulated environments were used to assess the preservation of the immobilized enzyme in the gastrointestinal tract. The results showed that ß-gal immobilization with 1% (w/v) sodium alginate presented the best results (EM of 66%, Y of 41%, and ar of 65%). The immobilization system maintained the highest pH stability in the range between 5.0 and 7.0, with the highest relative activity obtained under pH 5 conditions. The temperature stability was also favored by immobilization at 50 °C for 30 min was obtained a relative activity of 180.0 ± 1.37%. In 6 h, the immobilized ß-gal was able to hydrolyze 46% of the initial lactose content. For the gastrointestinal simulations, around 40% of the activity was preserved after 2 h. Overall, the results described here are promising for the industrial applications of ß-galactosidase from K. lactis.

Introdução: A eritrodermia esfoliativa é um evento raro que está associado a diversas doenças como psoríase, eczemas, neoplasias malignas, uso de medicamentos, entre outras. Relato do caso: Paciente de 63 anos apresentou quadro de exantema difuso iniciado em janeiro de 2018, evoluindo com descamação generalizada da pele e formação de úlceras, sentindo calafrios. Não referiu histórico de câncer na família. Na anamnese, não houve relato de nenhum tipo de eczema, doença de pele preexistente ou uso de plantas medicinais. Foi identificada neoplasia maligna de mama do subtipo molecular luminal A, posteriormente. Após mastectomia com linfadenectomia, houve apenas melhora parcial do quadro da eritrodermia. Atualmente, em uso de doxorrubicina. Conclusão: A paciente apresentou quadro clínico inicial condizente com o que se espera de eritrodermia, que pode estar associada ao surgimento de neoplasia maligna de mama. O presente relato é importante, pois pode auxiliar em diagnósticos diferenciais para a eritrodermia, mesmo na vigência de um quadro clínico atípico.


Introduction: Exfoliative erythroderma is a rare event associated with several diseases such as psoriasis, eczema, malignant neoplasms, medication use, among others. Case report: A 63-year-old patient presented diffuse rash that started in January 2018, evolving with generalized skin desquamation and ulcer formation, with chills. Did not report family history of cancer. In the anamnesis, there was no report of any type of eczema, preexisting skin disease or use of medicinal plants. Malignant breast cancer of luminal A molecular subtype was identified later. After mastectomy with lymphadenectomy, there was only partial improvement in erythroderma. Currently using doxorubicin. Conclusion: The patient presented an initial clinical condition consistent with what is expected from erythroderma, which may be associated with the emergence of malignant breast neoplasm. The present report is important because it can help in differential diagnoses for erythroderma, even in the presence of an atypical clinical case.


Introducción: La eritrodermia exfoliativa es un evento raro asociado con varias enfermedades como psoriasis, eccema, neoplasmas malignos, uso de medicamentos, entre otros. Relato del caso: Paciente de 63 años presentó una erupción cutánea difusa que comenzó en enero de 2018, que evolucionó con descamación generalizada de la piel y formación de úlceras, con escalofríos. No informó antecedentes familiares de cáncer. En la anamnesis, no hubo informes de ningún tipos de eccema, enfermedad cutánea preexistente o uso de plantas medicinales. La neoplasia de mama maligna del subtipo molecular luminal A se identificó más tarde. Después de la mastectomía con linfadenectomía, solo hubo una mejoría parcial en la eritrodermia. Actualmente usa doxorrubicina. Conclusión: La paciente presentó un cuadro clínico inicial consistente con lo que se espera de la eritrodermia, que puede estar asociada con la aparición de neoplasma maligno de mama. El presente informe es importante porque puede ayudar en los diagnósticos diferenciales de eritrodermia, incluso en presencia de un cuadro clínico atípico.

The 2019 coronavirus pandemic has united scientific and medical communities in a worldwide quest for understanding the pathophysiology of this rapidly spreading disease in order to develop effective treatments. We present a case of a 46-year-old woman with breast cancer who was found positive for SARS-CoV-2 in a screening test and developed massive rhabdomyolysis (creatinine kinase 87,456 U/liter) as well as new-onset lymphopenia and signs of lung disease starting on the 16th day of clinical surveillance, one month after the last administration of chemotherapy. Nasopharyngeal swab was still positive for SARS-CoV-2 RNA and serology revealed antibody response against the virus. Considering the possibility of a systemic inflammatory response in the setting of post-chemotherapy immune reconstitution, we avoided aggressive fluid administration and initiated treatment with methylprednisolone and hydroxychloroquine, resulting in rapid clearance of pulmonary infiltrates and creatinine kinase. Complete resolution after corticosteroid treatment may provide clinicians with a viable treatment option in similar situations and adds to the growing body of evidence pointing to dysregulated immune response as a major contributing factor to disease severity.

The present study evaluated the co-production of ß-galactosidase and ethanol by Kluyveromyces marxianus ATCC 36907 and Kluyveromyces lactis NRRL Y-8279 using as carbon source the lactose found on "coalho" cheese whey. Cheese whey was subjected to partial deproteinization, and physicochemical parameters were assessed. Cultivations were carried out in an shaker to evaluate two carbon/nitrogen (C:N) ratios. The best C:N ratio (1.5:1) was carried to 1.5-L bioreactor cultivation in order to increase co-production yields. The stability of ß-galactosidase was assessed against different temperatures and pH, and in the presence of metal ions. Concerning the co-production of ß-galactosidase and ethanol, K. lactis proved to be more efficient in both the C:N ratios, reaching 21.09 U·mL-1 of activity and 7.10 g·L-1 of ethanol in 16 h. This study describes the development of a viable and value-adding biotechnological process using a regional cheese by-product from Northeast Brazil for co-production of biomolecules of industrial interest.

This study proposes the investigation of curcumin encapsulation into Saccharomyces cerevisiae cells through osmoporation as an efficient way of increasing curcumin stability. The influence of three process parameters (cell, ethanol and curcumin concentrations) on the encapsulation process was evaluated, and the obtained biocapsules were characterised for physical and photochemical stabilisation. Results showed that encapsulation efficiency was favoured by the increase of cells/curcumin ratio and ethanol concentration up to 60%. Differential scanning calorimetry (DSC) curves revealed that yeast encapsulation delayed the curcumin melting point up to 207 °C. Encapsulated curcumin retained over 80% of antioxidant activity after thermal treatment (150 °C) and over 70% after a 50-day exposure to artificial light. Photochemical stability of yeast-encapsulated curcumin was increased by 5.7-fold, and half-life time reached 181 days under illumination conditions. Overall, osmoporation-produced yeast biocapsules confirmed the versatility of osmoporation as an encapsulation technique and successfully improved curcumin stability.

A amiloidose cardíaca, uma doença rara, pode cursar com insuficiência cardíaca por infiltração das paredes atrial e ventricular, dificultando também sense e limiares adequados quando do implante de marcapasso. Relatamos o caso de uma paciente que apresentou curso atípico, com rápida infiltração da parede atrial e perda da função ventricular, apesar da apresentação inicial com ecocardiograma e ressonância magnética normais. O caso ilustra o acometimento progressivo da doença de base em diversos aspectos cardíacos, inicialmente no sistema de condução atrioventricular, progredindo para maior fibrose atrial e acometimento miocárdico difuso. Os métodos complementares de ecocardiograma e ressonância magnética aumentaram o grau de suspeição da doença e nos permitiram fazer o diagnóstico específico


Cardiac amyloidosis, a rare disease, may lead to heart failure caused by infiltration of the atrial and ventricular walls, making it difficult to obtain sensed atrial stimulation and adequate thresholds at the time of pacemaker implantation. We report the case of a patient with an atypical outcome, rapid atrial wall infiltration and loss of ventricular function, despite the initial presentation with normal echocardiogram and MRI. This case illustrates the progression of the baseline disease, initially in the atrioventricular conduction system, leading to greater atrial fibrosis and diffuse myocardial involvement. The complementary echocardiogram and magnetic resonance imaging improved the level of suspicion of the disease and enabled us to make a specific diagnosis

Este estudo tem por objetivo apresentar a análise de Gilbert Ryle (1949/2009)do funcionamento lógico dos conceitos relacionados à emoção, incluindo sentimentos, inclinações, agitações e estados de ânimo. Ryle não propõe uma nova teoria das emoções, e sim uma análise de como os conceitos funcionam em seu “lar original”. Por meio desta leitura guiada da análise de Ryle, busca-se oferecer um exemplo concreto e desfazer confusões frequentes sobre o papel desse tipo de elucidação conceitual, mostrando que ela é condição prévia para o estabelecimento das condições de sentido de perguntas empíricas. Antes de fazer parte de teorias psicológicas, os conceitos de emoção nascem e se transformam nos contextos das interações cotidianas, dos quais é inseparável o entendimento das regras lógicas para seu uso. Quando nos esquecemos disso, há o risco de formular revisões conceituais inadvertidas que levam a confusões na teorização e a perguntas de pesquisa que não podem, logicamente, ser respondidas empiricamente.(AU)


We present Gilbert Ryle’s analysis (1949/2009) of the logical operation of concepts related to emotions, including feelings, inclinations, agitations and moods. Ryle does not propose a new theory of emotions, but an analysis of how these concepts work in their “original home”, which is ordinary language. Through this guided reading of Ryle analysis, we seek to offer a concrete example and undo frequent confusion about the role of this kind of conceptual clarification, by showing that it is a prerequisite for establishing the conditions of sense of empirical questions. Before becoming part of psychological theories, emotion concepts are born and transformed in the contexts of human interaction which establish criteria for their use. When we forget this, we risk inadvertently revising concepts which may lead to confusion in theorizing and research questions that cannot logically be answered empirically.(AU)


Se presenta el análisis de Gilbert Ryle (1949/2009) de la operación lógica de los conceptos relacionados con las emociones , incluyendo sentimientos, inclinaciones , agitaciones y estados de ánimo. Ryle no propone una nueva teoría de las emociones , pero un análisis de cómo funcionan estos conceptos en su “lugar de origen” , que es el lenguaje ordinario. A través de esta lectura guiada de análisis de Ryle, buscamos ofrecer un ejemplo concreto y deshacer frecuente confusión sobre el papel de este tipo de clarificación conceptual, demostrando que se trata de un requisito previo para el establecimiento de las condiciones de sentido de cuestiones empíricas. Antes de convertirse en parte de las teorías psicológicas, conceptos de emoción nacen y se transforman en los contextos de interacción humana, los cuales establecen criterios para su uso. Cuando nos olvidamos de esto, corremos el riesgo de adoptar cambios conceptuales inadvertidos, lo que puede dar lugar a confusiones en la teorización y a preguntas de investigación que no pueden lógicamente ser solucionadas de forma empírica.(AU)

Gastrointestinal tract involvement in immunodeficiency-related Burkitt's lymphoma is not common and the duodenal involvement is very rare. We report the case of a 35-year-old man admitted because of abdominal pain, vomiting and weight loss. Human immunodeficiency virus infection was diagnosed and upper digestive tract endoscopy showed marked edema and hyperemia of the duodenal bulb with some violaceous areas. Immunohistochemical study of the bulbar tissue samples confirmed the diagnosis of Burkitt's lymphoma. To our knowledge, duodenal Burkitt's lymphoma affecting only the bulb has not been previously reported in the medical literature. In patients with human immunodeficiency virus infection who present with upper gastrointestinal symptoms, upper endoscopy may be diagnostic of malignancy and biopsies should be obtained from abnormal areas.

RESUMO Investigou-se o efeito da leitura dialógica - leitura em voz alta intercalada com perguntas e feedback baseados em dimensões temáticas (funções) da narrativa - sobre a compreensão de contos dos Irmãos Grimm. Vinte e dois contos foram lidos individualmente a três crianças, sendo duas com oito anos de idade e uma com treze, de forma simples (sem intervenções adicionais) e dialógica, em um delineamento de linha de base múltipla por participante. A compreensão foi avaliada por meio da porcentagem de eventos do enredo e funções da narrativa verbalizadas em tarefas de reconto. A correspondência entre as histórias e os recontos foi superior na condição Leitura Dialógica, especialmente para funções narrativas. Discute-se o papel das perguntas, do reforçamento diferencial e do texto nos efeitos encontrados.


ABSTRACT We investigated the effects of dialogic reading - reading aloud interspersed with questions and feedback based on narrative thematic dimensions (functions) - on the comprehension of brother Grimm’s tales. Twenty-two tales were read individually to three children (aged 8, 8 and 13 years), in a baseline condition (Straight reading) and intervention (Dialogic reading), using multiple baseline design across participants. Comprehension was assessed through percentage of story events and narrative functions verbalized during retelling tasks. Correspondence between tales and retelling was higher under the Dialogic Reading condition, especially for narrative functions. The role of questions, differential reinforcement and text are discussed.

RESUMO

A doença de Camurati-Engelmann é uma displasia diafisária progressiva, autossômica dominante, caracterizada por progressiva expansão cortical e esclerose, que afeta, na maioria das vezes, as diáfises dos ossos longos, associada à hiperostose craniana. As manifestações clínicas são dor óssea nos membros inferiores, fraqueza muscular e atrofia muscular. Os autores relatam um caso de doença de Camurati-Engelmann em paciente masculino, de um ano de idade com diagnóstico avaliado pela radiografia simples, tomografia computadorizada, cintilografiaóssea e exames laboratoriais durante oito anos de evolução da doença. Ressalta-se a importância do diagnóstico diferencial com outras doenças que cursam com hiperostose e o emprego de corticosteroide para controle do quadro álgico.


Camurati-Engelmann disease is a rare progressive diaphyseal dysplasia, autosomal dominant, characterized by progressive cortical expansion and sclerosis mainly affecting the diaphysis of the long bones associated with cranial hyperostosis. The clinical manifestations are lower limbs bone pain, muscle weakness and muscular atrophy. The authors report a case of Camurati-Engelmann disease in a 1-year-old male patient who was submitted to plain radiography, computed tomography, bone scintigraphy and laboratory exams for eight years. The authors emphasize theimportance of the differential diagnosis with other diseases that attend hyperostosis and the use of corticosteroids to control the pain.

Foi avaliado o efeito da suplementação energética sobre o desempenho e as características de carcaça de 24 novilhos cruza Aberdeen Angus x Charolês terminados aos 24 meses em pastagem de azevém anual (Lollium multiflorum L.) e aveia (Avena strigosa Schreb) manejada em condições de oferta de forragem não limitante ao desempenho animal. Os tratamentos foram níveis de 0; 0,4; 0,8 e 1,2 por cento PV de oferta de concentrado à base de milho moído e minerais. Os animais foram abatidos à medida que atingiram um mínimo de 4,5mm de espessura de gordura na garupa (P8), avaliado por meio de ultrassonografia. Não foram observadas diferenças significativas sobre a deposição de gordura, avaliada in vivo por ultrassonografia ao final da etapa de desempenho, e sobre o ganho médio diário, que apresentaram valores médios de 3,9mm e 1,54kg d-1, respectivamente. A avaliação dos parâmetros bioquímicos séricos evidenciou redução linear dos níveis de ureia e aumento linear para os níveis de colesterol, sem que diferenças significativas fossem observadas sobre os níveis de glicose e triglicerídios. A suplementação produziu incremento linear no rendimento de carcaça e no ganho médio diário de carcaça. Entretanto, não foram observadas diferenças significativas sobre a área de olho de lombo, o peso de abate e o peso de carcaça quente, que apresentaram valores médios de 67,85cm², 438,95kg e 227,75kg. Os níveis crescentes de suplementação utilizados produziram incremento no rendimento e ganho médio diário de carcaça, sem alterações significativas sobre os demais parâmetros de desempenho e carcaça avaliados.


The impact of energy supplementation on the performance and carcass caracteristics of 24 Aberdeen Angus x Charolais steers grazing an annual ryegrass (Lollium multiflorum L.) and Oats (Avena strigosa Schreb) pasture managed in order to avoid constrains to animal performance was evaluated. Treatments were levels of supplementation of 0; 0.4; 0.8 and 1.2 percent BW with a corn and mineral based supplement. Animals were slaughtered as they achieve 4,5mm of fat cover in the rump (P8) evaluated ultrasonography. No differences were observed in fat deposition, measured in live animals with ultrasound at the end of performance period, and in live weight gain, which showed average values of 3,9mm and 1,54kg d-1 respectively. Blood serum levels of urea were linearly decreased and cholesterol linearly increased by energy supplementation, with no differences in the serum levels of glucose and triglicerides. Energy supplementation linearly increased the killing-out proportion and carcass weight gain, but no differences were observed on the rib eye area, slaughter weight and hot carcass weight, wich averaged 67,85cm², 438,95kg e 227,75kg. Increasing the levels of supplementation resulted in higher killing-out proportion and higher carcass daily gains, without changes in performance and carcass parameters evaluated.
